California's CalAIM reform expanded Medi-Cal to include Enhanced Care Management (ECM) and Community Supports — housing navigation, medically tailored meals, sobering centers, recuperative care, and more — for the state's highest-need members. The hard part is reach: identifying eligible members, building trust, and walking them through enrollment and follow-through.
Futurenomic deploys community health workers and culturally matched outreach teams that meet members where they are. We coordinate with managed care plans, FQHCs, behavioral health networks, and ECM providers to move people from outreach to enrollment to actual care. Perinatal community support and doula services are in active development.
