Futurenomic deploys community health workers who look like the neighborhoods they serve and stay on the case until care is actually delivered.

What is CalAIM and why does it need philanthropy?
CalAIM is California's Medi-Cal reform. It expanded coverage to include Enhanced Care Management and Community Supports such as housing navigation, medically tailored meals, sobering centers, and recuperative care. The benefits exist; the reach does not. Finding eligible members, earning their trust, and walking them through enrollment is unfunded relationship work, and that is where private gifts do the most good.
Who does Futurenomic serve in this work?
Medi-Cal members with the highest needs, including justice-involved residents, unhoused and housing-insecure members, and BIPOC communities in under-resourced regions. Perinatal community support and doula services are in active development.
Who do you coordinate with?
Managed care plans, FQHCs, behavioral health networks, and Enhanced Care Management providers. We move people from outreach to enrollment to actual delivered care, and we share data across our health, workforce, and youth programs.
Is my donation tax deductible?
Yes. Futurenomic Resources is a 501(c)(3) nonprofit organization. Cash gifts from US taxpayers are deductible to the extent allowed by law, with a receipt emailed immediately.
We are a health plan or foundation, not an individual donor. How do we engage?
